What is the process to get a religious exemption in your state?
There are two ways to claim a non-medical vaccine exemption in Oregon.
1. Watch the Vaccine Education Module for children in child care, preschool, or K-12 and print out the Vaccine Education Certificate at the end of the module. Then fill out the non-medical exemption section of the Certificate of Immunization Status form (pdf). Then the parent needs to turn in both forms to your child’s school or child care provider.

2. Talk to a health care provider and have them sign a Vaccine Education Certificate. Meet with a health care provider and get a signed copy of the Vaccine Education Certificate. Then the parent needs to fill out the non-medical exemption section of the Certificate of Immunization Status form (pdf). The parent then turns in both forms to your child’s school or child care provider.

What health care can minors consent to on their own?
A minor who is 15 years of age or older may give consent, without the consent of a parent or guardian, to hospital care, medical or surgical diagnosis, or treatment by a physician, dentist, physician assistant, or nurse practitioner.
A health care provider administering COVID-19 vaccines may not require parental or guardian consent for a minor who is 15-17 years of age and is exercising his or her right to consent to treatment. Also, a COVID-19 vaccine administration site may not force the health care provider to require parental or guardian consent in such cases.
