What is the process to get a religious exemption in your state?
An affidavit signed by a legally authorized representative stating that the immunization conflicts with the tenets and practices of a recognized religious denomination of which the student is a member and then submitted to the school.
For childcare programs ONLY:
Copy of written statement from the parent or guardian must state the reason(s) why the parent or guardian does not wish to have the child immunized submitted to the daycare center.

What health care can minors consent to on their own?
A minor may consent to examination and treatment for sexually transmitted diseases, including prophylactic treatment. The consent or notification of the minor’s parents or guardian is not required, but they shall be liable for the expenses of any treatment to minors under their custody.
If the minor has a sexually transmitted disease, the local health director may request that the minor be interviewed by an investigator to secure the names of sexual contacts for the purpose of locating and eliminating sources of infection.
The physician shall incur no civil or criminal liability for conducting a diagnostic examination or rendering treatment, but immunity shall not apply to any negligent acts or omissions. The physician shall incur no civil or criminal liability for any adverse reaction to medication given, if reasonable care has been taken to determine any history of sensitivity or previous adverse reaction to medication with patients under 20 years of age.
