What is the process to get a religious exemption in your state?
For a medical exemption: The Certificate of Medical Exemption (Form IMM51) is available through your physician. The form must be submitted to the school, before a child can be enrolled.
For a religious exemption: A Certificate of Religious Exemption (Form IMM52) can only be issued by a county health department. The parent or legal guardian must submit a written objection. A child may not attend school until the appropriate Immunization Form is provided to the School.

What health care can minors consent to on their own?
Any minor who is 14 years of age or older; or a minor who has graduated from high school, is married, divorced, or pregnant; may give effective consent to any legally authorized medical, dental, health or mental health services for himself or herself, and the consent of no other person shall be necessary. A minor may also consent to services to determine the presence of, or to treat, pregnancy, venereal disease, drug dependency, alcohol toxicity, or any reportable disease.
